Why Review Your Notes

While PatientNotes generates highly accurate clinical documentation, reviewing your notes is an essential step in the documentation process. AI-generated notes are designed to assist you, not replace your clinical judgment.

Always Review Before Finalizing

You are responsible for the accuracy of your clinical documentation. Review all AI-generated content before adding it to your patient's medical record.

Quick Review Checklist

  • Patient identification: Verify any patient details are correct
  • Chief complaint: Accurately reflects the reason for visit
  • Clinical findings: Exam findings match what you observed
  • Medications: Drug names and dosages are correct
  • Assessment: Diagnoses and clinical reasoning are accurate
  • Plan: Treatment plan is complete and correct

Direct Editing

The simplest way to make changes is to edit the note directly. Click anywhere in the note to place your cursor and start typing.

The note editor works like a standard text editor. Click, select, type, delete—all the standard editing functions work as expected.

Editing Features

Text Selection

Click and drag to select text. Double-click to select a word. Triple-click to select a paragraph.

Copy & Paste

Use standard keyboard shortcuts (⌘C/⌘V) to copy and paste text within the note or from external sources.

Undo & Redo

Press ⌘Z to undo changes and ⌘Shift+Z to redo. Your edit history is preserved during the session.

Using the AI Assistant

For more complex edits, use the AI assistant bar at the bottom of the screen. Simply type what you want to change, and the AI will help you update your note.

Located at the bottom of the screen. Type natural language requests and the AI will modify your note accordingly.

Example AI Commands

Try these types of requests:

"Expand the HPI section"

Adds more detail to the History of Present Illness

"Add differential diagnoses to the assessment"

Includes relevant differential diagnoses based on the presentation

"Make the plan more concise"

Condenses the treatment plan while keeping key information

"Add follow-up instructions"

Adds standard follow-up and return precautions

"Change the diagnosis to Type 2 Diabetes"

Updates specific content based on your correction

"Add a review of systems section"

Generates a new section based on transcript content

Be Specific
The more specific your request, the better the result. Instead of "make it better," try "add more detail about the patient's pain characteristics" for targeted improvements.

Common Edits

Here are the most common types of edits users make to their generated notes:

Adding Missing Information

Sometimes details are missed in transcription or don't make it into the note. Add them manually or ask the AI to include specific points.

Correcting Terminology

Fix drug names, dosages, or medical terms that may have been misheard or incorrectly transcribed.

Adjusting Assessment

Refine diagnoses, add clinical reasoning, or adjust the assessment to better reflect your clinical judgment.

Expanding/Condensing Sections

Use the AI assistant to add detail where needed or trim sections that are too verbose for your documentation style.

Ensure documentation supports the level of service billed—add complexity, time documentation, or additional elements as needed.

Saving Your Changes

PatientNotes automatically saves your edits as you work. You don't need to manually save—changes are preserved in real-time.

Auto-Save Enabled

Every edit is saved automatically. You'll see a brief "Saved" indicator after changes. No need to worry about losing work.

Version History

PatientNotes maintains a history of your note versions. If you need to go back to a previous version:

Access previous versions of your note from the version history
  • Each generated version is saved separately
  • Use the version selector to view previous generations
  • Major edits create restoration points

Undoing Changes

Made a mistake? You have several options to undo changes:

Keyboard Undo

Press ⌘Z (Mac) or Ctrl+Z (Windows) to undo recent edits.

Regenerate

Generate a fresh note from the transcript to start over.

Previous Version

Select a previous version from the version history.

AI Assistant

Ask the AI to "undo the last change" or "restore the assessment section."

Best Practices

Review systematically—start at the top and work through each section rather than jumping around.

Use the AI for bulk changes—if you need to rephrase entire sections, the AI is faster than manual editing.

Make direct edits for small fixes—typos and minor corrections are faster to type than to describe to the AI.

Trust your clinical judgment—if something doesn't sound right, change it. You know your patient best.
