How Note Generation Works

PatientNotes uses advanced AI to analyze your transcript and generate structured clinical documentation. The AI understands medical context, extracts relevant information, and organizes it according to your chosen template format.

Selecting a Template

After recording, switch to the Notes tab and select a template for your documentation. The template determines the structure and format of your generated note.

Available Template Types

SOAP Note

Subjective, Objective, Assessment, Plan format. The most common format for outpatient visits.

H&P (History & Physical)

Comprehensive format for new patient visits, including detailed history sections and ROS.

Progress Note

Streamlined format for follow-up visits focusing on interval changes and treatment progress.

Specialty Templates

Psych Eval with MSE, Cardiology, Pediatrics, and other specialty-specific formats.

Choosing the Right Template
Select the template that matches your visit type. A follow-up visit works better with a Progress Note than a full H&P. When in doubt, SOAP is a safe default for most outpatient encounters.

Generating Your Note

Once you've selected a template, click "Generate" to create your clinical note. The AI processes your transcript and produces structured documentation.

Generation Time

Note generation typically takes 10-30 seconds depending on the length of your transcript and complexity of the visit.

What Happens During Generation

  1. Analysis: The AI analyzes your transcript to identify clinical information
  2. Extraction: Key details are extracted: symptoms, findings, diagnoses, plans
  3. Organization: Information is organized according to your template structure
  4. Formatting: The note is formatted with proper medical terminology and style
  5. Review: The completed note appears for your review

Viewing Your Generated Note

After generation, your complete clinical note appears in the Notes tab. Review the content to ensure accuracy before exporting.

Understanding Note Sections

Generated notes typically include relevant sections based on your template and the content of your conversation:

Chief Complaint

The primary reason for the visit, extracted from the patient's initial statements

History of Present Illness

Detailed narrative of the current problem, including onset, duration, and associated symptoms

Physical Exam

Examination findings mentioned during the encounter

Assessment & Plan

Your clinical assessment and treatment plan as discussed during the visit

Generating Multiple Notes

You can generate different note types from the same recording. This is useful when you need documentation in multiple formats.

Regenerating Notes

If you want to try a different template or regenerate with the same template:

  1. Click on the template selector
  2. Choose the same or different template
  3. Click "Generate" again

Previous versions are saved, so you can compare different outputs.

PatientNotes saves each generated note version. You can switch between versions or go back to a previous generation if needed.

Tips for Better Notes

Verbalize your exam findings during the encounter ("Lungs clear bilaterally") for more complete documentation.

State your assessment clearly ("I think this is...") to help the AI identify your clinical reasoning.

Review the transcript first and correct any major errors before generating for the most accurate notes.

Choose the right template for your visit type to get the most relevant structure.

Common Issues

The note is missing information I discussed

Check your transcript to see if the information was captured. If it was, try regenerating the note. If it wasn't in the transcript, the audio may not have captured that portion clearly. You can add missing information manually when editing the note.

The note includes incorrect information

This usually stems from a transcription error. Check the transcript for the source of the error, correct it, and regenerate. You can also edit the note directly to fix issues.

The note is too brief or too detailed

Try a different template—some templates produce more concise output than others. You can also use the AI assistant to expand or condense specific sections after generation.

Generation is taking a long time

Longer transcripts take more time to process. Very long sessions (over 60 minutes) may take up to a minute to generate. If generation seems stuck, refresh the page and try again.
